Complete Forms Online for In-Person
If you have scheduled an initial assessment with the counseling center, you must complete a series of intake paperwork. This informs our practice and helps us provide the best form of care. Please click this link to complete your paperwork.
Once you click the link, select the tab that reads, "COUNSELING Intake - First Appointment", to complete your forms. Please complete all forms, or you will be asked to complete them during your appointment. When you are finished, the computer will return to the home screen and you are free to exit the site. Please plan to complete all documentation in one sitting, as the website times out after 10-15 minutes to protect the confidentiality of your responses.
Weaver Wellness Resource Center
We are pleased to offer access to resources for any student regardless of whether they receive clinical services through the Counseling Center. The following items may be checked out for a two-week period by any EMU student:

- RainLax Breathing Light: This breathing tool can help you reduce stress, increase focus, boost productivity, and achieve a deep, restful sleep at night.
- Light Therapy Lamp: Emits light which provides a non-invasive and safe way to combat winter blues, jet lag, shift work or seasonal changes, and boost energy.
